Personal and Educational Leave

Employee Self Service

Use remote access to log in to Employee Self Service and review and/or change your benefits information.


A Personal Leave requires an absence from work for 10 or more business days. Approved leaves hold no job protection and will terminate your benefits at the end of the month in which your leave is effective. Cobra will be offered to you as a continuation of coverage. If you return to work within 30 days of your first day out, your benefits will reinstate with no lapse in coverage.

Requesting a Personal or Education Leave 

Download the Leave of Absence Application and present to your supervisor for authorization and submit to your HR Benefits Administrator for processing.

*You must contact your HR Benefits Administrator upon your return to discuss the reinstatement of your benefits.
